Hum i have quite a list but by far of the most sad moments in anime, movies, books, and games here are as listed.

Clannad : Death of Nagisa
After Tomoya works so hard much struggle to keep her healthy and well he just looses her after giving birth, all his fears become a reality since he loved her so much.To Add the fact there was nothing he could do since she had to give birth stuck at home without proper medical attention.
# 2 Clannad : Death of Ushio
Ushio is Tomoya 's daughter,she was suffering from the same illness as her mother and eventually she died as Tomoya had lost everything trying to keep her alive, his job, his life,his belongings, everything, to go so far as a father truly moved me to the bitter ending the story sadly had.
# 3 Old Movie: Legend Of The Waterfalls
The scene where all brothers go to war ad of of em is murdered while the other only can do is helplessly watch as he struggled to save his life.
# 4 F F X
Ending scene where Tidus Defeats *bleep bleeeeep*and then has to *bleeeeeeep* ,after defeating *Bleep*
# 5 Code Geass : Shirleys Death.
Trully a masterpiece in anime, no words can describe.
# 6 Samurai X: Trust & Betrayal : Ending
Kenshin fell in love with Tomoe without knowing she was working undercover to help kill him. She wanted to kill him because he killed her fiance. Making him fall in love with her was sort of a tool to help weaken him when she breaks his heart. She didn't want to be in love with him but she kinda does. Well, he finds out the whole betrayal thing and goes looking for her. He gets badly beaten and stuff before he gets to the final boss battle. He was pretty much blind, deaf, and all sorts at the time he cut her in half. She was in the line of fire because she wanted to help him.
Never forget the old days.

Cry? No. But the ending of Ga-Rei: Zero was pretty sad.
The two clannad events mentioned early were pretty sad too, but the fact that in the end we get a "And they all lived happily" ruins it for me.
The end of "Gladiator" always gets me too, regardless of how many times I watch the damn movie

I'm more prone to get teary when there's nothing sad going on at all; rather it's the little heartfelt bits in fiction that triggers it. That said, it's the sad bits that really get me glued to the page/screen, but it's more of a rueful smile reaction.
Anyway, some of my favorite emotionally-charged scenes from various stories:
Naruto: The Shippuuden arc concerning Kakuzu and Hidan was a winner in my book; it's got a very nice death scene, and Shikamaru gets some great character development - the series would be a lot better if the arcs concerning the main characters were more like this one.
Higurashi no Naku Koro ni: Every episode or two, you get one of these, but my favorites are the scene between Shion and Satoko in Arc 5 (you know the one), the Rena/Keiichi fight in Arc 6, and the entire ending sequence in the penultimate arc of Kai.
Umineko no Naku Koro ni: Same case as its spiritual predecessor; favorites so far have been the George/Shannon proposal scene in Arc 2, and the deaths of Jessica and Kanon also in Arc 2.
My very favorite?
Tales of the Abyss: Anise Tatlin goes through most of the game as a pretty minor character as far as party members go, but when the spotlight finally reaches her, the result is beautifully painful. Anyone who's played the game or watched the anime will know what I mean. Anyone who hasn't should do themselves a favor and check it out.
