Get a Gamer mouse with the ball on the thumb. Its shaped differently and it positions your hand in a different way then a normal mouse does. Since I switched to it I've never had problems playing. Also you don't need to fish hours on end. Try to limit it to a few short runs every few days. Yeah is mentally agonizing but you'll get over it.

*edit* Also how about farming blue herbs to use instead of fish. Go find a ranker and have them brew you up a batch, they heal quite nicely !

For your hand right now, soak it in epsom salt, you'll find it in the first aid isle. It helps with swelling, bruising and the likes. It should help you recover faster.
