Well, you can only change the size with the Setup.exe so, did you make sure your options were still changed after you closed your setup ? Checking a second time isn't bad.
Another solution you could use it to use ROExt, as it allows you to set any size you want to the game. It requires diving a bit into configurations, but it's not really hard to understand.

Right click on Setup.exe and go into properties, set it to run as administrator, hit Ok.
Open Setup.exe by double-clicking it, change your options and hit Ok, it should have saved it. Make sure the client is closed when you change the options.
If you want a more custom solution depending on your screen's resolution.. from the iROwiki (this is what I did):
1. Open up "regedit" (type this into your windows search)
2. If you are running 32-bit follow this path:
HKEY_LOCAL_MACHINE\SOFTWARE\Gravity Soft\Ragnarok
If you are running 64-bit follow this path:
HKEY_LOCAL_MACHINE\SOFTWARE\Wow6432Node\Gravity Soft\Ragnarok
I have tried out the different options that has been posted except for the ROExt option suggested by Fyrus as I couldn't find the file.
The setup.exe file seems to be unrelated to the Hero client in both administrator account or the account I am using.
It was a bit of a hassle but I figured out how to change my window size using MonkeyPunch's suggestion. First, I edited the height and width values through my administrator account. Then I had to run the client as administrator in my user account. And finally the results panned out.
I'm using Windows 7 and running RO through a user account rather than the administrator's account.
